The Aged Care Facilities I have worked at seem to have very general policies on moving and handling ( they are often still called manual handling policies and come under health and Safety ) Some places have specific rules under Procedures eg a hoist must be used to lift non weight bearing residents from the floor.
I would suggest you base the policy on what is suggested in the Moving and Handling of People the NZ guidelines including the process of Risk assessment ( page 304 and 305 ) Its a good idea to include the facilities Mission Statement in the Policy if they have one.
I am working a few hours at a new Aged Care facility so will have a look at their policy on Thursday
